COVID-19 Vaccinations
Lincoln Trail District Health Department will be offering the Pfizer and Johnson and Johnson COVID-19 vaccines at each health center.
a. You must be 12 and older to receive the Pfizer vaccine. The Pfizer vaccine requires 2 doses 21 days apart. A parent/legal guardian must accompany all minors to provide consent for vaccination or designate an adult to accompany their child via the consent form. b. You must be 18 and older to receive the Johnson and Johnson (J&J) vaccine. The J&J requires only one dose to be considered fully vaccinated. For more information about the Johnson and Johnson vaccine, click here.
Individuals should contact their local health center to schedule an appointment.
LTDHD HEALTH CENTERS
Be ready to show proof of identity, age, and residency. LTDHD staff will ask for this documentation. The inability to provide proper identification will result in being turned away.
- Once you arrive, remain in your vehicle and call the appropriate number above.
- Please wear short sleeves to assist in the ease of vaccine administration.
- Be prepared to remain on-site for 15-30 minutes after receiving your vaccine to be monitored by LTDHD staff for any adverse reactions.
